FYI - SD cards and other solid state storage does not need to be defragged. Ever. All that does is shorten the life span of it. They have a limited amount of read/write operations before they start to fail. Usually, that's many years.
Defrag is meant for data stored on a rotating platter that has a mechanical arm that has to move around.
At most, I would suggest formatting the card in the camera.

Does the computer ever assign a drive letter to the memory card? Your reader could be defective if it never assigns a drive letter. If it's a SD card they come preformatted with Fat32. If you have one that's a SDHC, you can reformat it to NTFS so you'll be able to utilize the full size of the card.
Go into Computer Management by right clicking Computer-Manage. Then from there go to Disk Management and look to see if your computer ever assigns it a letter. If not, try to assign one yourself and see what happens.

I had this same problem. I put the memory card in my laptop and defragged the drive that the card shows up on. Seems to have fixed the problem for me. Good luck.
